When restoring, is it common practice to remove and replace all the nuts and bolts - Obviously the worn ones have to go, but just curious. If yes, does anyone have a list of all the sizes and Qtys?

I would say no. If the bolt is in good shape I'm not sure why you would unless you didn't want to re-finish it, or needed something longer/shorter due to modification, or need it to be stronger for some reason. The real purists are always hunting for good original/NOS bolts since many have the CDDP logo on the head, which a replacement won't have.
For mine if the bolt is in good shape I'll just clean it up and powder coat or paint the head or nut and re-install it. Especially for machined parts since I know it's the right size and length, and sometimes the design is non-standard.

Nearly every nut and bolt was too far rusted on mine to reuse. With the exception of the fasteners on both doors, I replaced every thing with grade 8 hardware. I'm working on an M43 now, and will most likely do the same thing with it, but haven't really decided for sure. So far the ones I've removed are in pretty bad shape, and I thinking I don't really want 2 different kind of fasteners showing.
